Brazil study finds effective treatment for dark spots, acne and redness
A new review of clinical trial data has found that basic skin care practices, including gentle cleansing, moisturizing, and sun protection, can produce meaningful improvements in common skin conditions, even without prescription ingredients.
The narrative review, published in Skin Health and Disease, examined the non-medicated control groups of randomized controlled trials. These groups, called vehicle arms, received a base formula without active prescription drugs. Researchers from the University of Lancashire and NHS Foundation Trust dermatology departments searched medical databases such as MEDLINE and Embase for relevant trials.
The review focused on five conditions: acne, actinic keratoses, melasma, post-inflammatory hyperpigmentation, and rosacea. The researchers then looked at what the vehicle arms achieved on their own.
Results by condition
For acne, two large phase III trials with a combined 2,817 participants showed up to a 25.7% improvement in overall facial acne severity and up to a 51.2% reduction in inflammatory lesions in the vehicle group alone.
A trial of 468 participants with actinic keratoses, precancerous skin spots caused by sun damage, found up to a 24% reduction in lesion count and 17% complete clearance in the vehicle arm.
For melasma, a trial of 48 participants showed up to a 66.6% improvement in severity with vehicle treatment alone.
A phase IV trial of 123 participants with post-inflammatory hyperpigmentation, commonly called dark spots, found up to a 44.9% improvement in overall disease severity with vehicle treatment.
For rosacea, a multicentre trial of 120 participants showed up to a 27% reduction in redness in the vehicle arm.
Why the findings matter
The vehicle arms delivered results from basic skin care steps: cleansing, moisturizing, and sun protection. The review’s authors noted that vehicle arms in clinical trials often achieve a substantial portion of the efficacy seen in active treatments, frequently exceeding half. This means baseline skin care can provide more than half the benefit of prescription treatments.
The researchers pointed out factors that can influence these results, including the Hawthorne effect, where people take better care of themselves when they know they are being observed, the placebo effect, and the fact that trial participants often follow routines more consistently than in everyday life. The numbers should be interpreted with nuance.
Even with those considerations, the core finding remains that foundational skin care is a meaningful intervention on its own, not just a baseline before active treatment begins.
A simple, consistent routine with a gentle cleanser, a good moisturizer, and daily sun protection is effective according to this research.
